INFORMATION FOR ADMITTED APPLICANTS

ENROLLMENT FORMS:
In addition to your College Net online enrollment form, there are additional forms and waivers that must be submitted in order to complete your enrollment in ESAP.
Please download and complete the following REQUIRED forms. Completed forms must be uploaded to your College Net dashboard (DO NOT EMAIL FORMS). The upload fields can be found by clicking on “Manage Your Account” and then click “View Checklist”.
- Summer Participant Agreement
- Parent Permission Form
- Summer Waiver and Release Form
- Authorization For Medical Treatment Form
- Proof of Medical Insurance (copy of both sides of your insurance card or policy statement; please upload as a single pdf)
*If necessary, international students may purchase a travel insurance policy (from a provider of their choosing) to cover them while in Philadelphia participating in the program.*
- Gym Waiver Form (16 years and older ) – OPTIONAL
*Please note that only students 16 years of age and older are able to access the gym (with a signed waiver). These regulations are set by the university and exceptions cannot be made under any circumstances.*

**Students arriving by CAR on July 9th must arrive to campus and check into the dorms between 12:00PM – 4:00 PM EST (12:00 – 16:00). Students must be moved out of the dorms by 11:00 AM EST (11:00) on July 29th. Free ground transportation is available for students arriving at Philadelphia International Airport between 8:00AM – 4:00PM EST (8:00 – 16:00) on July 9th. ALL ENROLLMENT FORMS MUST BE SUBMITTED NO LATER THAN Friday, MAY 26TH.
DEPOSIT PAYMENT:
Your non-refundable deposit payment is due at the time of enrollment. Your enrollment form and deposit payment must be submitted NO LATER than the date listed on your decision letter. After completing your CollegeNet online enrollment form, you will automatically be routed to the deposit page for payment. If you experience any issues with the enrollment or deposit payment page, please contact CollegeNET directly.
PROGRAM PAYMENT:
Please note that bills for ESAP will not be generated until late May. An email notification will be released during that time. Once you are enrolled in the university’s system in your respective ESAP course, a bill will be generated by Student Registration & Financial Services. Please note that our office DOES NOT handle billing, and all applicable inquiries must be made directly to SRFS. The balance due (minus the amount of your deposit payment) will be due on the date specified on your bill.* If you received a financial aid award, this amount will be deducted from your balance as well. If you financial aid award is not posted at the time your bill is generated, please do not worry – it will be posted and will be deducted from the overall balance. The university must receive the full amount due on or before the bill’s due date. If full payment is not received by the due date, a late penalty of 1.5% of the amount past due will be assessed, and your enrollment in the program may be jeopardized.

INTERNATIONAL STUDENTS:
If you are a not a US citizen, permanent resident, or current F-1 visa holder studying in the US, you WILL need an F-1 visa to participate in ESAP. We will be contacting all admitted international students with further details on applying for the F-1 visa. No action can be taken regarding your F-1 visa until WE contact YOU. Please be patient. Please note, visitor or tourist visas will not be accepted for this program.
